Our mental wellness programs takes place over 8 weeks with weekly one-on-one appointments with a qualified mental health specialist to provide personalized support and guidance to help you achieve your wellness goals around mental health. Through a combination of evidence-based practices, therapeutic elements, and mindfulness, our programs are designed to empower you to make lasting positive changes in your life. Over these transformative 8 weeks you explore your identity, challenge your thoughts, and deepen your understanding of wellness.

Wellness Program for Aging Adults


Our Mental Wellness Program for the Aging Adult is catered to adults and older adults who have experienced a major life change or transition (like a change in health, shift in situation or circumstance, or experienced a loss) and want to take control back over their emotions and overall wellness.

The wellness topics change week to week and are tailored to the individual. The program covers the following topics:

  1. Goal Setting & Psychoeducation on Stress & Anxiety
  2. Exploring Sense of Self & Building Resiliency
  3. Mindfulness & Coping Skills
  4. Social Support, Boundaries, & Communication
  5. Grief, Gratitude, & Ambiguous Loss
  6. Introduction to Cognitive Behavioral Therapy (CBT)
  7. CBT Continued
  8. Self-Care & Moving Forward

Our Wellness Program for Caregivers focuses on the unique challenges and concerns that caregivers face in their roles, not only as a support for others but as a independent identity as well.

Scheduling & Pricing


Weekly sessions are scheduled based on a time and date agreed upon by the coach and the client. Sessions are flexible to meet your needs and time constraints.  

Pricing is for the program in its entirety and is billed before the first appointment.

Our 8 Week Mental wellness Programs are only $400 and include 8 weekly one-on-one appointments with a qualified mental health specialist, an E-Workbook, and additional supportive resources.

Meet Rachael Hornbostel, LSW


Rachael has her Master’s in Social Work from the University of Denver and her Bachelor’s of Arts in Psychology from Agnes Scott College. She is a Licensed Social Worker in the state of Colorado and pulls from her extensive education and experience in mental health.

Rachael has over 6 years of experience working closely with adolescents, adults, and older adults. She found her passion for brain health and older adults while completing her master’s where she worked closely with survivors and their families to provide education, support, and rehabilitative services through in-person and virtual format. 

Rachael received her YTT200 yoga teacher certificate in 2018 and has had a personal practice for over 15 years. She enjoys the mind-body connection that yoga offers in conjunction with evidence based practices from her mental health training and education.

Rachael combines these skills and resources to provide clients with a personalized holistic health approach to increase quality of life and overall satisfaction.
